Welcome to the practice of Drs. Branton and Jarrah. We would like to thank you for having the confidence in allowing us to participate in your medical needs. Our goal is provide you and your family timely, respectful and the best quality medical care possible in a pleasant practice environment. The following is a review of our office policies.
The receptionist is generally present 8:30 a.m. to 5:00 p.m. Monday through Friday, except holidays. The front office will be closed from 12:00 p.m. to 1:00 p.m. daily for lunch. When the office is closed, a message can be left on the answering machine or in the event of an emergency, the physician on-call may be paged.
The physicians hold office hours from 8:30 a.m. to 4:30 p.m. Monday through Friday. Although our schedule is generally full daily, if you are sick or have an urgent problem we usually are able to see you that day. If you cannot or do not plan to keep your appointment, please let us know 24 working hours in advance to avoid a failure to cancel charge of $35.00.
If you have an immediate or life threatening emergency, call 911 or go promptly to an Emergency Room for assistance.
After Hours / Weekends / Holidays
Drs. Branton or Dr. Jarrah can always be reached (for emergencies only please) by simply calling the office and speaking to the answering service operator. The on-call physician will then be paged. We ask that you keep your phone line free so that the physician may call you back.
Prescription Refills and Referrals
Prescriptions are generally refilled during scheduled follow-up office visits. However, for your convenience, we also offer a telephone prescription refill line. Simply call the office during regular business hours and follow the menu system to access this feature. You may also request a prescription refill by visiting our Website at WWW.BJPA.Net . All refill requests made by 2:00 p.m. will be called in that day. Requests made after 2:00 p.m. will be called in the following day. Prescriptions for Controlled Substances (Narcotics, Ritalin, Adderall) cannot be refilled by phone and can only be dispensed during regular office hours. We also offer a similar Referral Line, which can be accessed via the phone menu system. All referrals require 72 hours prior notice for completion. Referrals should be picked up at the office prior to your specialist appointment.
Record Copying and Form Completion
A charge of $19.00 is required for document preparation plus a fee of no more than 63 cents per page copied plus the actual cost of shipping and handling. A completed and signed release of information form is required prior to release of records. All forms, i.e., applications, insurance, FMLA forms etc., may be subject to a reasonable fee for completion. Payment will be expected prior to completion of these forms. Physical exam forms presented at the time of your appointment will be exempt from this charge.
Ours fees are developed in consideration of our costs of operating the office as well as monetary return to the physician rendering the service. We feel that our fees are reasonable and we do expect to be paid. If your insurance contract requires a co-payment you will be required to pay this fee at the time of service. This fee cannot and will not be waived. As a courtesy to you, we will bill most major insurance plans and we suggest that you check with the receptionist as to whether we accept your particular insurance. We will also attempt to bill claim balances to your secondary insurance however, if claims are not paid in a timely manner, this balance will be turned over to your personal account for prompt payment. If we do not participate in your health insurance plan, a receipt of service will be provided for you to submit for reimbursement. Personal checks, cash, Master Card, American Express and Visa are accepted. There is a $25.00 fee on all returned checks. In the unfortunate situation where your account is left unpaid for 120 days or greater, your account will likely be turned over to our collection agency. Should this occur not only will you be responsible for full payment of the balance of your account, an additional 35% of your balance will be added to your bill in order to settle the collection agency fee.
We do expect you to know the details of your insurance contract, as this is a signed agreement between you and your insurance carrier. Since we deal with numerous different insurances, we cannot possibly know the details of each plan. Should you have a question of this nature, please contact your insurance representative.
Laboratory / X-Ray ResultsWe ask that you allow 10 business days for notification of all laboratories, x-ray results, etc. You will either be notified by letter or by personal phone call. You will be notified of any urgent result as soon as the report is available. If you do not hear from us within 10 days, please contact us and let us know.
